I understand that I will be offered an appointment based on the availability I have provided and that all slots offered are first come first serve. Once I have been contacted with an appointment offer I have 24 hours to secure it by paying a booking fee via direct bank transfer. If I fail to secure the offered slot and it is no longer available I will have to request an alternative appointment date.

A non refundable booking fee is required to secure my booking once a slot has been offered, and that it can only be transferred twice before I will need to pay it again. I will lose my booking fee if: I cancel my appointment; if I ask to reschedule less than 48 hours before my appointment; If I am more than 15 minutes late as this will cause my appointment to be automatically cancelled.

I may not be accepted for future bookings if I don't show up, am consistently late to appointments or reschedule too many times.

I must be at least 18 years of age to get a tattoo.

Any custom designs will be drawn up 24 hours before my scheduled booking, this includes the night before the appointment. If I request any changes to the sizing/placement/design requested on this form, or if I exceed the allowed time slot during my tattoo appointment I may be required to pay an extra charge.
